Risk assessment with people who have viewed CSA images

Risk assessment is not a one-off event, but rather a process that needs to be undertaken throughout therapy. Furthermore, risk assessment should include someone other than a therapist. Regrettably, deceit and denial are common in all forms of addiction and hence therapists must not assume they are hearing the full story. What’s more, one of the core personal requirements for being a therapist is having a natural tendency towards unconditional positive regard and empathy, which means we may not want to hear that our client is doing something wrong. Supervision is vital for any therapist, but when working with offenders it is even more critical. And if possible, someone outside of the organisation who is proficient within the field of offending should also be sought for consultation.

It’s also essential to be aware that on-going risk assessment will be required when working with those who are already within the criminal justice system, as well as those who are not. When a client presents whom the authorities already know, this can create a false level of comfort for the therapist who may assume the police, probation or social services hold the responsibility for risk. But as a therapist, you may hear additional information that the authorities are not aware of which you may have a responsibility to report; for example, if probation conditions or child protection plans are being breached.

It is not the aim of this chapter to provide a thorough risk assessment protocol, and indeed it could be dangerous to do so, as external verification should always be sought as explained above. However, the following are areas of exploration that can be helpful and also guide a clinician towards making the decision on how to work with a client or whether to refer.

1 How old are the minors in the images that have been viewed?

2 What was the nature of those images, for example were they still naked images or moving images of a child being forced into a sexual act?

3 How long has the client been viewing these images and how regularly?

4 Has the client ever shared CSA images?

5 Does the client have any history of violent, anti-social or offending behaviour?

6 Has the client ever tried, online or offline, to initiate a sexual relationship with a minor?

7 Does the client have any significant psychological problems?

8 Does the client have any concurrent addiction problems that might further disinhibit behaviour, such as alcohol addiction?

9 Does the client have contact with any children of a similar age to those they were viewing?

10 How does the client feel about the images they have seen?

11 Does the client demonstrate motivation for change, beyond not getting caught or reducing a possible sentence?

12 What external authorities are involved and is the client willing to let you liaise with those authorities?
